Diabetes covers the facts and dangers of type two diabetes, daily management including checking glucose levels and how to support self-management and referring patients to self-management programs.

5Asthma

Asthma covers asthma facts, causes, diagnosis, monitoring asthma control, management including asthma medications, addressing environmental triggers, asthma action planning and supporting children with asthma at home and in school.
